Correcting physical plant issues is how craft employees measure supervision's commitment to safety

For the most part craft people feel safety issues too often "fall through the cracks" while Supervisors feel that most issues are timely corrected

Methods for reporting safety concerns vary by location

• Directly to a supervisor
• Division Safety Hotline
• Location-specific written forms

Employees should use whichever available method they are most comfortable with

Regardless of reporting method, all safety issues should be entered on the division SIRP log

2 SIRP protocols

• Immediate correction (24-48 hours)
• Extended correction (more than 48 hours)

Each log entry should contain the following information

• Date the issue was reported/logged
• Brief description of the issue
• Owner of the issue (supervisor who received it)
• Corrective action plan that includes a timeline
  (extended protocol issues only)

  After correction and the item is marked closed

• A brief description of the corrective action
• Date corrected

Employee initiating a safety concern should be notified

• When the issue is received
• When the issue is corrected

Log should be posted on the Safety bulletin board

Log should be discussed during job safety briefings

Log is to be reviewed at Safety Committee meetings
